Sign up for a membership at Evolution!

To sign up for a membership at Evolution you will need to print the following documents and then:

1. Read, sign and date Member Agreement.
2. Read the Cardlock Policy.  Keep for your records.
3. Fill out Payment Option sheet.

Bring Member Agreement and Payment Option sheets into the fitness center during staffed hours, pay your membership fee, and we'll take your photo and give you a card!  Don't forget to schedule your free initial training consultation!
